Fees and Insurance

Please review the Informed Consent documents when starting services with us for a complete list of related fees not covered by insurance, such as report writing, IEP/504 meetings, court fees, etc.

Out-of-Network Insurance

We accept Caresource and Buckeye (medicaid) plans, as well as UnitedHealth Care, Aetna, & Anthem**.

We are considered out of network/private pay providers for other insurances. If we do not accept your insurance as form of payment, you can pay for therapy sessions with cash, check, credit card, or your HSA/Flex Spending Account card. Services will be billed directly to you according to our private pay therapy rates listed above at time of service and will be collected at the time of service.

We can provide you a "superbill" for each service if we are out of network with your insurance. Superbills can be submitted to your insurance provider for possible reimbursement or application to your deductible. Please contact your insurance provider to determine reimbursement rates.

**Anthem has a significant amount of subplans and we cannot guarantee we will be in network with each subplan. Please call and follow the insurance benefits checklist prior to starting services to look at your specific plans’ benefits.

Insurance Benefits Checklist

There is a 24 hour cancellation policy. No show/late cancel will be charged a full session fee, unless there is an emergency or otherwise discussed with your therapist. This is not covered by insurance.

Good Faith Estimate

All clients have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost starting January 1, 2022. A Good Faith Estimate enumerates the expenses you can reasonably expect to pay for your mental health care services provided by Wired to Bloom.
